Wellness and Chronic Disease Programs

The Greater Johnstown Community YMCA (Y) offers a variety of Wellness and Chronic Disease Programs to members of all ages so that they might enjoy a more healthy and fulfilling life.

Our trained professionals can get you acquainted with our facility, help you kick-start your fitness journey, and assess your individual needs to create a customized program that will help you reach your goals. Our areas of expertise include:

  • Weight-loss
  • Strength and Mobility
  • Aging Populations

In addition to Wellness Coaching (personal training), we offer Sports-Specific Coaching for a wide variety of sports including Basketball, Swimming, and Track & Field. All Wellness and Sports Coaching sessions are one-hour-long with the exception of swim instruction, which are 40 minutes in length.

Community-Based Chronic Disease Programing

In communities across the nation, the Y is a leading voice on health and well-being. For over 15 years, the Y has driven innovation in prevention and control of chronic disease to improve quality of life, save lives, and reduce healthcare spending. With support from the Centers for Disease Control and Prevention, the Y delivers tangible results in scientific evidence-based chronic disease prevention and control. Across America, Ys are increasing opportunities and policies promoting physical activity, expanding health and wellness initiatives, and offering health care solutions and education outside the traditional healthcare model.

The Greater Johnstown Community YMCA is proud to offer a wide-variety of chronic disease prevention and control programs and meeting the expanding needs of our region. Taking a community-based approach to health care, our team is able to deliver medical care and education in a community setting at low or no cost as part of our mission to combat chronic diseases in our region.

Blood Pressure Self-Monitoring Program

Take Action to Improve Heart Health

If you are living with high blood pressure, taking action to keep it well-controlled will minimize your risk for complications like stroke, heart attack, and even death. High blood pressure, which makes the heart work harder than it should with each beat, is especially dangerous because it often has no warning signs and usually lasts a lifetime once it develops. The simple process of checking and recording your blood pressure at least twice a month over a period of four months has been shown to lower blood pressure in many people with high blood pressure. This forms the basis for the YMCA’s Blood Pressure Self-Monitoring Program, an evidence-based program that features personalized support as you develop the habit of routinely self-monitoring your blood pressure, tips for maintaining your cardiovascular health, and nutrition education.

How the Program Works

Participants will work with trained wellness coaches for the duration of the four-month program. Just a few check-ins per month can yield big results. You will:

• Take your own blood pressure at least two times per month
• Attend two consultations with a Wellness Coach per month
• Attend monthly nutrition education

You will record your blood pressure readings using an easy-to-use tracking tool of your own choosing. The Y can provide options for participants who need a home blood pressure monitor.

A Small Investment Produces Big Results

The YMCA’s Blood Pressure Self-Monitoring Program helps participants:

• Reduce their blood pressure by identifying patterns and trends on which to take action
• Better manage their blood pressure over time
• Increase their awareness of triggers that elevate blood pressure
• Improve their knowledge of heart-healthy eating habits
• Enrich communication with their health care provider by sharing self-monitoring results

Program Features

Wellness Coaches

Wellness Coaches will train you on the proper technique for taking your own blood pressure readings accurately, will assist you in identifying trends and patterns in your readings over time, and will provide ongoing support as you work to make blood pressure self-monitoring a habit.

Office Hour Consultations

Wellness Coaches are available each week for one-on-one 10-minute consultations to support you as you continue to better manage your blood pressure. You’ll be asked to attend at least two consultations a month, but you will be welcome any time a Wellness Coach is on-duty. To ensure you feel supported throughout the program, you will also receive weekly emails, calls or text messages from a Wellness Coach.

Nutrition Education Seminars

Seminars will highlight how food is related to blood pressure management and will provide you with the knowledge and skills to make heart-healthy choices. Seminar topics include reducing sodium intake and heart healthy eating. Each 60-minute seminar will also offer an opportunity for you to take your own blood pressure with coaching on proper technique from an on-site Wellness Coach.

Our Goal:

Reduce an individuals risk of experiencing a cardiac event by providing positive guidance for healthy living and the appropriate level of physical activity.

What is Community-Based Cardiac Rehabilitation?

Community-based cardiac rehabilitation (Phase 4) provides people with known coronary heart diseases the opportunity to benefit from weekly supervised exercise sessions. People who are eligible include those who have participated in Cardiac Rehabilitation programmes and those who meet certain medical criteria and have been assessed by a physician.

What to Expect

  • Two exercise session each week for one month
  • Exercise monitored by a certified exercise physiologist or personal trainer.
  • Designed to reduce the risk of another cardiac event and help to prevent existing conditions from worsening.
  • Risk factor modification.
  • Emotional support.
  • A physicians note is required for this program.

Free Wellness Areas Orientation

This is a free program to new and existing members. The orientation will get you started on the path to success! One of our trained Wellness Staff members will lead you on a tour of our Wellness Areas, including our Free Weight Room and Nautilus Room. You will learn where equipment is, how to use the equipment safely, and best practices.

After you have completed your orientation we invite you to use your knowledge to work towards your goals. If you know you are looking for a more personalized approach or simply want to learn more, our staff are always here to help and we invite you to enroll in our Free Wellness Coaching Orientation!

Free Wellness Coaching Consultation

After you have taken our Wellness Areas Orientation we invite you to meet with one of our trained Wellness Staff members, who will sit down with you to discuss your health history, past injuries, training history, and your specific health and wellness goals. This free consultation session will introduce you to goal setting, healthy eating habits, and the basics of strength and conditioning programming.

The Wellness Coaching Consultation will provide you with basic information and tools to get you on your way. If you are interested in working with one of our Wellness Coaches check out our Private and Semi-Private Wellness Coaching options.
